How it works

Submit the assistance request below (or call Customer Service).
Our team will follow up to arrange the healthcare-provider certification that assistance is needed. If you would rather take the form to your provider yourself, download the Sanitation Disability Service Form (PDF) and return it by mail, email, or secure fax — the return details are on the form.
Once certified, bin assistance is added to your regular service.
